Altered Android app icon, indication of spyware?

While setting up and debugging a VPN connection on an Android device with the “Net Analyzer” app, I spotted by chance a suspicious icon in the application as shown in the screenshot below (shows the Chinese flag instead of a gear icon).

The icon is still there even if I remove the application and do a re-install from the official GooglePlay store. Interestingly, if I install the “Net Analyzer” app on another device, the application displays the usual expected icon, see 2nd screenshot.

Additionally, I checked the suspected device with a malware checker, which did not report any incidents.

Prior to performing a factory reset of the device, I wanted to crosscheck with the community:

  1. Is a changed icon a known indication of a compromised device or just a UI issue?
  2. Is there any experience with “Net Analyzer” to be somehow linked to spyware?
